Owners
Nick and Desiree Trunzo are proud to bring Town Square to Folsom, California—a community they know and love from growing up and living in the Greater Sacramento region. They are passionate about providing a modern, meaningful solution to the growing needs of local seniors and their families. Together, they lead with heart, integrity, and a genuine commitment to doing right by every family who walks through the door. They provide a place where seniors feel connected, families feel supported, and community truly thrives.
With more than a decade of experience managing operations and developing high-performing teams, Nick and Desiree have built their careers around caring for others. Over the past several years, they have focused on programs and services that help seniors remain independent and age safely at home. Town Square is a natural extension of that mission—offering a safe, engaging, and joyful environment where seniors can stay active, build friendships, and enjoy each day with purpose.
Center Director
Jillien has worked in senior and long-term care for 28 years, across settings that include in-home care, adult day programs, residential living, and skilled nursing. She previously served as the administrator for the El Dorado County Older Adult Day Program in El Dorado Hills for eight years. Her primary areas of focus include high-quality staff training, equipping families and care partners, educating community members, and ensuring her programs consistently deliver person-centered care and meaningful life enrichment.
She is a Certified Alzheimer’s and Dementia Care Trainer, a Certified Dementia Practitioner, and a Certified Activity Consultant. She holds a degree in Psychology with a minor in Gerontology. Her passion is supporting seniors with chronic illnesses and helping their families maintain the highest possible quality of life throughout the course of the disease.
Drawn to Town Square by its shared commitment to innovative and compassionate care, Jillien specializes in addressing challenging dementia-related symptoms such as resistance to personal care, late-day confusion, anxiety, and wandering. As a dementia family support group facilitator and care-partner coach for more than 20 years, she is skilled in providing understanding, guidance, and education to families in both group and one-on-one settings.
A dynamic and engaging public speaker, Jillien enjoys offering community education on topics such as safety in aging, brain health, and the complexities of dementia. She is honored to bring her expertise to Town Square—supporting exceptional daytime care services and providing ongoing education to care staff, families, and the broader community.
Program Director and Center Nurse
Melissa Nace is a registered nurse with over 20 years of clinical experience, specializing in home health and hospice care. She has dedicated her career to supporting patients and families through diverse health challenges and disease processes, providing compassionate, person-centered care during some of life’s most difficult moments.
Deeply passionate about dementia care, Melissa finds great meaning in creating programs and environments that honor memories and joyful moments. She is excited to bring the Town Square service model to life, helping members reconnect with positive memories and experience comfort, dignity, and engagement.
Center Dietitian
Sharon Lim is a Certified Nutrition Specialist (CNS) with six years of experience in health and wellness, following a 12-year career in the corporate world. She specializes in root-cause healing, with a focus on how nutrition, epigenetics, and ancestral health patterns impact chronic disease, including cognitive decline and brain health in aging adults.
Sharon holds her CNS credential and brings a whole-person, compassionate approach to her practice – integrating clinical nutritional science with the lived experience of her clients to support lasting, meaningful change.
She is drawn to Town Square’s commitment to dignity, joy, and meaningful connection for older adults – values she believes are at the heart of true nourishment. This work also carries personal meaning; her husband’s family has been touched by memory loss, and that reality has deepened her respect and care for those navigating this journey and the families who love them.
