Meet the Perry Hall Team

Ashley Diaz, RN began her nursing journey in 2018 as an LPN, working in long-term care and skilled nursing settings where she developed a strong passion for geriatric care. During this time, she gained hands-on experience caring for individuals with complex medical needs and learned the importance of patience, consistency, and compassion in supporting older adults She joined Town Square in 2022 and quickly found a sense of purpose in supporting older adults and their families in a more personal, meaningful way. After earning her RN in 2023, Ashley stepped into the role of Nurse Manager, where she now oversees clinical operations while remaining closely connected to the day-to-day care of members.

Keyonna Jordan is the Director of Operations and Programming for Town Square Perry Hall, an adult medical day center that was developed as an innovative alternative to standard day options. She is a professional with over 19 years of experience. Keyonna is a graduate of Beaufort Community College in Washington, North Carolina where she received her Activity Director certification, became a member of NCCAP (National Certification Council for Activity Professionals), and successfully completed MEPAP 1&2 (Modular Education Program for Activity Professionals) in Maryland as well as her ServSafe Food Service Manager certification. She has worked in both adult daycares and in long-term skilled nursing facilities. Keyonna is caring and reliable and is always reminded by her grandmother that she has found her calling. Keyonna lives by the motto “Activities is more than just bingo.”

Laura Dilworth is the Center Operations, Billing, and Compliance Specialist for Town Square Perry Hall. Laura graduated from the Community College of Baltimore County with an associate of the arts degree, continued with her education, and graduated from Vision Allied Health Institute in 2021 as a Certified/Geriatric Nursing Assistant. Laura began working as the billing and warranty specialist at Skytech Inc., located at Carroll County Regional Airport, where she gained expertise in billing, administration, and customer service. In 2021, she began to work at Town Square as a Program Assistant, where she found her passion for working in healthcare and helping seniors attain their utmost quality of life. Due to her passion, she returned to Vision Allied Health Institute in February of 2022 and completed the Certified Medication Technician certificate. Laura is currently in nursing school to become a Licensed Practical Nurse. However, she does not want to stop there, as her goal is to one day become a Nurse Practitioner with a specialty in memory care and geriatrics. She is looking forward to expanding her career in nursing and with Town Square. On weekends, Laura enjoys spending time with her young children.

Nichole is a compassionate and dedicated professional with more than 20 years of experience in leadership, human
resources, operations, and program management. She currently serves as an Activities Director, specializing in
engagement programming for adults with cognitive impairments, including Alzheimer’s and related dementia. In this
role, she develops and implements meaningful, person-centered activities that promote cognitive stimulation, social
interaction, and emotional well-being. She also supports staff training, assists with coordinating daily programming, and
ensures activities are adapted to meet varying levels of ability.
Throughout her career, Nichole has gained extensive experience in staff leadership, program development, client
relations, payroll, recruitment, and day-to-day operations across healthcare, nonprofit, and corporate environments. She
is recognized for her ability to lead teams effectively, support families, and foster safe, supportive environments that
enhance quality of life.
Driven by a passion for service, Nichole blends strong organizational leadership with a person-centered approach,
helping individuals maintain dignity, connection, and purpose while addressing the unique needs of adults living with
cognitive impairment.

Dr. Michael D. Hayes ll is a distinguished scholar, pastor, and healthcare executive whose career reflects a unique blend of academic excellence, spiritual leadership, and business expertise. He earned a Doctor of Philosophy (Ph.D.) in Philosophy, a Master of Divinity (M.Div.), and a Bachelor of Science in Economics, equipping him with a broad foundation in critical thinking, theology, leadership, and organizational management.

As an ordained pastor, Dr. Hayes has faithfully served the church and community, offering spiritual guidance, biblical teaching, and compassionate leadership. His commitment to ministry has impacted countless individuals through preaching, mentorship, and community outreach initiatives.

In addition to his pastoral work, he has built an accomplished career in the healthcare industry, serving for more than 12 years in leadership roles. As a Sales Director, he has demonstrated exceptional ability in strategic planning, business development, team leadership, and client relations, consistently driving growth and organizational success.

Dr. Hayes is also a proud member of, Kappa Alpha Psi Fraternity, Inc…where he upholds the fraternity’s longstanding tradition of scholarship, leadership, fellowship, and service. Through his professional achievements, ministry leadership, and community involvement, he continues to inspire others through a life dedicated to excellence, integrity, and service.
